Application for Supplemental Unemployment Benefits (SUB)

To file a claim for the SUB benefit, complete this application and return to the Fund Office with confirmation of payment from the State unemployment office.

Claim for SUB Benefits upon Separation from Service

Complete this form and return it to the Fund Office to apply to have your SUB Fund account balance paid out to you upon separation of service.

Claim for SUB Benefits upon Retirement

Complete this form and return it to the Fund Office to apply to have your SUB Fund account balance paid to you or direct it to be used to pay retiree health care premiums under the Electrical Workers Health and Welfare Fund.

FREQUENTLY ASKED QUESTIONS

Your account is funded solely through employer contributions. Your employer makes contributions to the SUB Plan based on the Local 294 Inside Construction Agreement.
Once you become a Plan participant, contributions made on your behalf are 100% vested- benefits in the Plan are yours, and generally cannot be taken away from you.
You are eligible for payment of your Accounts if you have had at least $750 in Employer Contributions made to your account and:
  • • You are receiving State unemployment compensation benefits
  • • You have separated from service from the electrical industry
  • • You have deceased

Yes, benefits are generally taxable the same as wages and you will receive a W-2 for benefits you receive.
To apply for a benefit you will need to complete an application and return to the Fund Office.
In order to change your mailing address, you must complete a Change of Address Form. You may print and complete the form, or you may contact the Fund Office and a form will be mailed to you.
